Tom's story

 

Tom is a 44-year-old man living independently in Bristol, working full time for the British broadcasting corporation and enjoying a busy social life. He frequently travels between Bristol and Birmingham to see friends and family.

Tom also manages several complex health conditions, including osteogenesis imperfecta (brittle bones), restrictive lung disease, Crohn’s disease and chronic kidney disease. The fragility of his bones, in particular, means he needs support with clinical interventions and parts of daily living.

Tom began his care journey with Pulse Nursing at Home more than ten years ago, shortly after relocating from Birmingham for work. Throughout this time, we have provided a carefully structured care package to support his clinical needs and protect his independence, so that he can continue living the life he wants safely and confidently.

What started as a practical decision has since grown into a trusted partnership or as Tom describes it, “one of the best decisions I’ve made.”

 

Our approach

At the start of our partnership, Pulse Nursing at Home worked closely with Tom and commissioners to design a bespoke 24/7 live-in care package. The aim was for him to be able to live independently while safely managing complex clinical needs.

Since then, his care has been delivered by a consistent, highly trained team who support with moving and handling, personal care, medication and respiratory monitoring, as well as daily routines such as preparing for work and evening care. The care is flexible, allowing Tom to maintain an active lifestyle, including work, socialising, traveling and time with family.

Continuity is central to Tom’s care. Long-standing relationships with his support workers have built trust, mutual understanding and strong communication. As Tom describes, it’s “like a housemate on steroids”, a unique balance where his team knows when to step in and when to step back.
