Commissioned care

We work directly with NHS commissioners, ICBs and case managers to build, staff and manage packages of care for clients with complex needs who want to live well at home and in their communities across England.

Every package is shaped around the person. From rapid response support to a lifetime of care, we take the time to get it right for the individual and for the families who love them.

Our complex and specialist care at home

Whatever the need, whatever the funding framework, we specialise in building complex care packages that are clinically led and centred entirely around the individual.

Care that works towards independence

Our aim is always to do more than just provide care. Through carefully considered interventions, we work with individuals to build the skills and confidence they need to regain independence, stepping down support gradually where possible and always at a pace that feels right for them.

It’s not just about the care we put in place today. It’s about the life we help people build for tomorrow.

– Patient’s next of kin

Working with us

We make placing a complex care package straightforward. Here is exactly what to expect from your initial referral right through to care delivery.

Our referral process

Initial discussion

The first step is an initial discussion to understand the client’s care requirements.

Proposal and costings

We’ll then put together a tailored proposal, outlining suggestions and care options along with costings for review.

Carry out assessment

Once you’ve reviewed and approved the proposal, your allocated clinical lead will arrange an assessment to fully understand the client’s needs.

Personalised care plan

Our clinical lead will develop a tailored care plan covering services, schedules and any special requirements. This may involve input from healthcare professionals and other parties to complete the necessary documentation.

Identify suitable workers

Our team of care coordinators will identify suitable workers and arrange the necessary training which will be aligned with the clients needs.

Agreement and contract

Prior to a package of care starting, we’ll require a contractual agreement in place. This will outline the terms and conditions of the services and any other relevant information.

Service commencement

Once the agreement is signed, the care plan and clinical documentation finalised and care workers have completed their training and sign-off processwe’ll work with all parties to agree on a suitable start date.

Ongoing support

We maintain regular communication and reviews with the client and care workers, providing updates, addressing concerns and adjusting the care plan as needed to ensure it continues to meet the client’s needs.

Our people

We focus on building and retaining a skilled team of nurses and support workers so that packages are safe and clinically robust from day one.

Robust recruitment

Every nurse and support worker is recruited with enhanced DBS checks, full referencing and a clinical competency assessment.

Ongoing training

Continuous supervision and training ensures our workforce remains equipped to deliver safe, high-quality complex care.

Clinical-led support

Every team is backed by clinical leads, service delivery managers and care coordinators to maintain safety and performance.

Case study

Supporting Marley and his family

Marley lives with Tay-Sachs, causing seizures and difficulty clearing secretions, so regular suctioning is essential.

Nights were once the hardest part for Marley’s mum, managing constant repositioning and seizures.

Since our nurses began coming into the home, nervousness turned to relief, with trained professionals watching over Marley and managing his care.

As Mum says: “They’re our lifeline. They’re like our family.”
