From intensive care to home – Dylan’s story

Dylan spent the first year of his life as an inpatient at Birmingham Children’s Hospital.

He was transferred to Acorns Children’s Hospice before we were approached to build a package of care to transition him home, in a highly rural location, with a family who had understandably low trust in care providers.
