We work with legal teams and private case managers pre and post-settlement, supporting cases with clear documentation and cost reporting along with delivering full packages of care.

Pre-settlement

We provide care package costings and assessments to support the litigation process. Our team produces evidence-based quotations that reflect the individual’s long-term care needs. Where care needs to begin before settlement, we can also put this in place.

Post-settlement

Once a settlement is reached, we implement bespoke packages of care in line with the individual’s needs, if our care isn’t already in place. We work with case managers to keep care plans current, reviewing regularly and adapting as the client’s circumstances change.

Making a referral is straightforward

Initial discussion

The first step is an initial discussion to understand the client’s care requirements.

Proposal and costings

We’ll then put together a tailored proposal, outlining suggestions and care options along with costings for review.

3. Carry out assessment

Once you’ve reviewed and approved the proposal, your allocated clinical lead will arrange an assessment to fully understand the client’s needs.

Personalised care plan

Our clinical lead will develop a tailored care plan covering services, schedules and any special requirements. This may involve input from healthcare professionals and other parties to complete the necessary documentation.

Identify suitable workers

Our team of care coordinators will identify suitable workers and arrange the necessary training which will be aligned with the client’s needs.

Agreement and contract

Prior to a package of care starting, we’ll require a contractual agreement in place. This will outline the terms and conditions of the services and any other relevant information.

Service commencement

Once the agreement is signed, the care plan and clinical documentation finalised and  care workers have completed their training and sign-off process, we’ll work with all parties to agree on a suitable start date.

Ongoing support

We maintain regular communication and reviews with the client and care workers, providing updates, addressing concerns and adjusting the care plan as needed to ensure it continues to meet the client’s needs.
